Effective 1/1/25 Parkview Physician Group and any remaining Parkview Community Hospital will be out of network for Lutheran Preferred and Three Rivers Preferred. Enrollment in your employer's health plan is described in the employer's benefit plan. The benefit plan is administered by a Third Party Administrator (TPA) that has been selected by the employer.
